ANNOUNCEMENT FOR DR.HADLEY'S PATIENTS
Dear Patients and Colleagues,
For over 30 years I have been fortunate to share in the lives of patients and colleagues and for this I am forever thankful. These relationships have so much shaped the person I am and enriched my life. Thank you!!
However, as the years pass it is time to move on and I will retire from St. Paul FHO effective July 1, 2026.
With the ever-changing face of primary health care in Ontario as my retirement approaches, the focus of leaving my patients in good hands has been paramount.
I am pleased and proud to transfer your care to Dr. Abiola Ogunyemi as he joins our wonderful team of physicians, nurses and staff at St. Paul FHO.
Dr. Abiola Ogunyemi has over a decade of clinical experience. He earned his medical degree in Nigeria, then moved to the United Kingdom, where he completed his postgraduate training in Family Medicine. Over the past two years, he has been practicing as a Family Physician in Ontario.
As part of the transition we will need to re-enroll you with Dr. Abiola Ogunyemi.
If you opt to transfer your care to another primary care provider, we can assist in providing a copy of your medical record to you or your new provider.
